Information we may collect from you

We may collect your personally identifiable information (such as name, address, date of birth, telephone number, e-mail address or other identifying information in relation to Covid-19 or other testing) only when you choose to submit it to us. You can visit our website without providing any personal information. We may collect information about you that you provide by responding to our questionnaires or surveys conducted from time to time for industry research purposes. We may collect your personal identifiable information when you request further services or when downloading documents from our website. We may also ask you for information when you report a problem with our website.

As part of providing a professional, safe and efficient service, there is certain information that we record. This includes details of drugs and appliances dispensed against NHS prescriptions as well as significant advice given, and referrals made to other health professionals and any other relevant information.

For the purposes of Covid-19 or certain other testing, results data may be required by law to be shared by us or our third party organisations in a timely manner to Public Health England and/or other public health authorities.

IP Addresses and Cookies

We may collect information about your computer, including where available your IP address, operating system and browser type, for system administration and to report aggregate information. This is statistical data about our users’ browsing actions and patterns and does not identify any individual.

For the same reason, we may obtain information about your general internet usage by using a cookie file which is stored on the hard drive of your computer. Cookies contain information that is transferred to your computer’s hard drive. They help us to improve our site and to deliver a better and more personalized service. They enable us to:

Transferring Information outside of the UK

We will not transfer the personal information we collect about you outside the UK.

No fee usually required

You will not have to pay a fee to access your personal information (or to exercise any of the other rights). However, we may charge a reasonable fee if your request for access is clearly unfounded or excessive. Alternatively, we may refuse to comply with the request in such circumstances.

What we may need from you

We may need to request specific information from you to help us confirm your identity and ensure your right to access the information (or to exercise any of your other rights). This is another appropriate security measure to ensure that personal information is not disclosed to any person who has no right to receive it.

Right to withdraw consent

In the limited circumstances where you may have provided your consent to the collection, processing and transfer of your personal information for a specific purpose, you have the right to withdraw your consent for that specific processing at any time. You will not be subject to decisions that will have a significant impact on you based solely on automated decision-making, unless we have a lawful basis for doing so and we have notified you.
